Call the Comcast support line. When the IVR asks you what to help with, say "Cable Card". You should get transferred directly to level 2, when they answer, say you need Cable Card support. They should then transfer you to the Cable Card dept. I have had much better success calling later at night than during the day.

After many calls and repeating the same information to multiple people, we eventually were told to go to the xfinity store and get a new cablecard. After installing and calling up to get it paired and restarting the TiVo it now works again. The process was not painless but at least things are working again for the time being.
